• Liaise with nurses to arrange home-care visits as necessary
  
• Arrange a Personal Service Worker to accompany you on your trip home
  
• Schedule follow-up care visits with you and ensure that you are on the road to recovery.
• Tailor a schedule for nutritional meal delivery through Meals On Wheels (if necessary)
